My CareSight provider takes the time to listen to me and go over all of my medications with me. I have COPD, diabetes, and arthritis, so it’s not easy to get to my appointments. By coming to my home, she makes it convenient for me.

Here’s What You Can Expect

  • 1 Initial Visit
    We match you a local nurse practitioner who will meet you in your home at a time that’s convenient for you.
  • 2 In-Home Assessment
    Your nurse practitioner will conduct a physical exam, review your medical history, go over treatments, order any tests, and review your medications. Your clinician will also do a safety check of your home, offer nutrition guidance, and answer questions about upcoming tests or procedures.
  • 3 Regular Check-Ins
    After initial visit, you can receive medical care at home whenever you need it, 24 hours a day. We’ll check in between your regular doctor visits too, and send results of every visit to your primary care physician.
